Pancoast syndrome is a malignancy of the superior pulmonary sulcus causing subsequent destruction of the thoracic inlet and involvement of the brachial and oculosympathetic plexuses.7 Most cases involve non-small cell lung carcinoma.

A Pancoast tumor is a variation of lung cancer. The tumor starts at the top left or right lung and goes into the chest wall. Pancoast tumors are also known as superior sulcus tumors. Pancoast syndrome is the term used to describe the specific symptoms that accompany this type of lung cancer. Pancoast syndrome is most commonly associated with a primary lung carcinoma and rarely with metastatic malignancies and certain infections, including mucormycosis. It is characterized by Horner syndrome, shoulder pain radiating down the arm, compression of the brachial blood vessels, and, in long-standing cases, atrophy of the arm and hand muscles.

Pathophysiology Pancoast or superior sulcus tumor consists of a wide range of tumors invading the apical chest wall and producing a characteristic syndrome named “Pancoast -Tobias syndrome.” The superior sulcus is an anatomical pleuro-pulmonary groove that is formed by the subclavian artery when it curves in front of the pleura and runs upward and lateral immediately below the apex. Horner's syndrome symptoms are the result of nerve damage and disruption.
